By accessing and using services provided by Lab X-Perts LLC, including scheduling appointments, submitting information, undergoing testing, or receiving results, you agree to be bound by these Terms and Conditions, including our obligations under the Health Insurance Portability and Accountability Act (HIPAA).
Lab X-Perts LLC provides in-lab and mobile diagnostic services, including but not limited to:
Drug & alcohol testing (DOT and Non-DOT)
DNA testing (legal, immigration, paternity)
Health screenings (STDs, COVID-19, flu, etc.)
Phlebotomy, gender reveal, and vision testing
Fingerprinting and background checks
Mobile collection services and compliance solutions
We reserve the right to update, suspend, or discontinue services without prior notice.
Payment is due at the time of service unless a contract or corporate billing agreement is in place.
We accept major credit cards, cash, and authorized third-party payments.
Cancellation within less than 24 hours may incur a cancellation fee.
No-shows may result in denial of future service without prepayment.
Lab X-Perts LLC is committed to protecting your personal health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and all applicable federal and state privacy laws.

We will never sell your information or use it for marketing without your written consent.
Data Security:
All records are stored in secure, HIPAA-compliant systems.
Mobile collections and paper forms are processed and destroyed in accordance with federal regulations.
All employees receive HIPAA training and are bound by confidentiality agreements.
Most results are delivered within 24–72 hours depending on the service.
Results may be sent securely via email, portal, or printed form.
Positive or inconclusive results may require confirmation testing or review by a Medical Review Officer (MRO)
Mobile collections are subject to availability and location. By booking a mobile service, the client agrees to provide a safe, clean environment and access to restroom facilities (if required).
DNA results intended for legal or immigration use must be collected following chain-of-custody protocols.
It is the client’s responsibility to inform us if results are needed for legal, court, USCIS, or employment purposes.
Fraudulent submission of a test on behalf of another person may result in denied service and legal reporting.
Lab X-Perts LLC is not liable for any incidental, consequential, or indirect damages resulting from use or reliance on our services. All test results are based on the sample provided and represent findings at the time of collection.
Lab X-Perts LLC reserves the right to revise these Terms and Conditions at any time. Updated versions will be posted on our website with an updated effective date.
